We agree to be bound by, comply with and observe the Club Rules and Codes of Conduct (available on request from the Club Chairman or Club Secretary) and additionally the Rules and Regulations of The Football Association and County Football Association for all competitions in which the Club participates.

We consent to disclosure by County Football Association.

We agree to be responsible for any club kit provided to the member and that we will reimburse the club should this not be returned.

We agree that these details may be kept on computer files and that player’s names and pictures may be published, either in paper or electronic format, wherever the club committee deem suitable

In the event that my child is injured whilst playing football/travelling to and from football events and I cannot be contacted on the above number, I hereby give my consent for my child to receive medical attention.

I give permission for my child to be given lifts to and from football events in other parents’ or helpers’ cars.

Full annual membership and league registration: £200.00
Training only membership for juniors (U7 to U18): £100.00
